The Benefit Book will recognize all Sponsors and Contributors by listing their name(s) at the level of their support.

The Benefit Book will be distributed to all attendees of the Benefit, mailed to donors of $300 or more, and will be available to everyone online.

RESIDENT PHOTO SECTION INFORMATION

If the total of the donations that are assigned to your MFA# is $1,250 or more, you may submit a photo of your resident, with caption, to be included in the Resident Section of the book.

CREDIT CARD PAYMENT INSTRUCTIONS:

Upon submitting this form, you will be taken to a confirmation page. You can pay by credit card from the confirmation page by clicking the “Click here to proceed to payment” link at the bottom.

This link will take you to the secure payment page for the MFA Benefit Book powered by GiveSmart™. You will need to provide your credit card information.

To make your contribution on behalf of a Misericordia resident, please enter the resident’s name or family name and the Misericordia Family Association Number (MFA#) in the text fields provided.

Then, enter the actual amount of your contribution. (This can be more than the amount of the level you chose.) Then, click on “Checkout”. You should receive an email from GiveSmart™ confirming your payment.
